Supervision privacy notice consent

Acorn Counselling Therapy (Peter Golder) treats all supervisees data with the upmost care. As part of this all therapy supervisees are requested to read the following privacy notice. I ask for your consent to hold information about you, that you give to me directly or that I learn about you as we begin to work together. If you agree, then enter your name, date and digitally sign this document below before the first session.

Personal information I collect 

  • Name and address

  • Telephone

  • Email address

  • Basic notes following each supervision session

  • Bank account number and sort code if paying by bank transfer

How I store personal information

Online

  • Supervision contract

  • Privacy notice

Smartphone/Laptop

  • Supervisees phone numbers are stored in a password protected phone along with the supervisee's name.

  • Brief session notes. These are stored securely.

Email

  • Email addresses and correspondence will be stored within our email account by nature of supervisees contacting us by this method.  This email account is password protected. 

Bank Details

  • If you pay by bank transfer, it is possible that your name would be on the bank statement of Acorn Counselling Therapy. Peter Golder (Therapist) will see these bank statements. These statements are online statements that are password protected.

How We May Process / Share Personal Information

Personal information

Only Peter Golder as the therapist will access any personal data.

Consultation

All UKCP/UKATA registered psychotherapists are required to seek monthly consultation with another therapist qualified in this process and bound by the same obligations with regards to supervisees confidentiality. This is to ensure good practice and allow therapists’ ethical standards to be monitored for the safety and well-being of people.  In order to maintain privacy, consultants will only be told a supervisee's first name.​

Deleting Information

As required by my insurance provider, I will keep session notes for up to seven years. After this time, these will be shredded if in paper form or deleted from all software packages on electronic devices.

Supervisees Rights

Supervisees have the right to:

  • Be informed about how and why your information is held (this privacy notice)

  • Request access to any information held about you

  • Request for information about you to be rectified

  • Restrict data processing

  • Request data portability

  • Withdraw consent to any information being used / held, although this may mean it is no longer possible to access therapy.

  • Request personal details to be erased, however in the case of session notes these may be retained for insurance purposes (as previously explained)

  • Not be subject to automated decision-making including profiling
